Service Agreement
Thank you for your interest in Rise Up Consulting Pte. Ltd (Rise Up). This form aims to clarify the framework of our work together. Rise up provides services for individuals with special needs, which include, but are not limited to, ABA individualized programs, parent education, and counseling. Employees and contractors hired by Rise Up to provide services will be referred to as “specialists” in this agreement. Feel free to contact us by phone or email should you have any questions. CONFIDENTIALITY
Your right to privacy and confidentiality is of the utmost importance to Rise Up. Hence, it is important to note the following: If we were to accidentally see each other in public, Rise Up employee(s) and contractor(s) will not acknowledge you. However, we will be more than happy to converse with you if you initiate interaction. Please note that we prefer keeping treatment-related discussions in a private space. The session content and all relevant materials/data to the client’s treatment could be shared with the supervisor in charge. Outside of supervision hours, we may need to consult with other professionals in their areas of expertise in order to provide the best services for you. Every effort will be made to avoid revealing your or your children’s identity by sharing information about you in this context without using your name. When not related to treatment consultations, such information will be held confidential unless the client provides written authorization to release all or portions of such content to specific individuals or agencies. Exceptions are made as required by law, or where allowed by law for a valid reason. This includes, but is not limited to: (1) providing needed services and obtaining appropriate consultations; (2) sharing treatment information with caregivers when the client is minor and; (3) protecting clients or others from harm.
FINANCIAL POLICIES
Rise Up aims to be transparent in its financial policies. Pricing for sessions and any other related expenses (e.g. assessments, reports, meetings) will be disclosed and agreed upon before the start of services. Fees are subject to change prior to a minimum month's notice. Fees are inclusive of GST. Invoices are billed every month. Unless explicitly agreed, the payment is expected within 7 days of the invoice date. Rise Up reserves the right to charge a late payment fee of S$25.
SCHEDULING/CANCELLATION
Sessions are typically scheduled weekly. This time is held for the client weekly in the specialist’s schedule. Hence, please remember to cancel or reschedule the sessions as early as possible. Rescheduling is preferred as it ensures the client gets consistent hours across months. However, Rise Up reserves the right to refuse sessions rescheduling should it not fit with the current schedule of the specialist. Cancellation fees are a necessity given the arduous nature of replacing missed sessions with another client. However, Rise Up understands that emergencies can arise and that a holiday may be needed once in a while. Hence the following exceptions apply:
● Given at least 4 hours' notice, 1 emergency cancellation (e.g. Sickness) is allotted per month without penalty.
● Sessions falling on 11 Public Holidays are also free of charge should you decide to not do a session on that day.
Outside of those exceptions, the following fees apply:
● The full fee will be charged if a session or consultation is canceled less than 24 hours in advance.
● After more than two consecutive weeks of cancellation (e.g. extended holiday), Rise Up can no longer guarantee your sessions unless they are paid for.
If the client isn’t present upon the start of a session (in person or online), the specialist will try to call you and wait 15 minutes before leaving the premises (cancellation fees will automatically apply). Furthermore, should you/your child arrive late, the session will not be extended past the allocated scheduled time. If the cancellation is due to the specialist (e.g. sickness or holidays), it will result in no penalty on your part. Rise Up will make every effort to reschedule appointments should the cancellation come from us.
SICKNESS
In order to protect clients, families, and staff, please notify Rise Up as much in advance as possible if you know that the client or people in your home are showing signs of sickness including temperature above 37.5°C, vomiting, diarrhea, lice, rash, etc. Parents are asked to use the same guidelines used in schools: if a child is too sick to attend school, he/she is too sick to participate in his/her session. If the specialist arrives at the home and judges that the client is too sick or contagious, the specialist reserves the right to leave the premises and the full price for the session will be billed. Sessions will resume as soon as a doctor clears him/her of being contagious or the client has been 24 hours symptoms free.
TERMINATION
It is important to have a proper termination process for transition and closure. Do advise Rise Up at least a month in advance before the termination of services. Rise Up reserves the right to terminate services without notice should you be in default on payment or should you fail to schedule an appointment for more than two weeks without explicitly agreeing. Please also note that if the client was to miss more than two consecutive weeks (e.g. extended holiday), Rise Up would be unable to guarantee your session’s times. Given the purpose of termination and if requested, Rise Up will work to provide you with a list of alternative services, therapists, centers, or schools that correspond to the needs of the client. CONTACTING Given our professional commitments, Rise Up might often not be immediately available by telephone. Rise Up will attempt to return calls or emails within 24-48 hours with the exception of holidays and weekends. In case of emergency/crisis situations, please call 999 and/or go to the nearest hospital emergency room.
